Switching your Home Care Provider

Five Easy Steps to Switching your Home Care Provider Easily

1. Do your research

Shop around and check that your new Home Care Provider provides the range of services to meet your current and future needs, has the ability to accommodate you and then learn the date when your services will commence.

2. Call My Aged Care on 1800 200 422

Or, log on to My Aged Care and let them know of your decision to relocate your Home Care Package.

Ask them to activate your referral code and supply the referral code to your new Home Care Provider before your services commence.

3. Notify your new Provider

When your referral code is accepted by your new Home Care Provider, your current provider will be notified through My Aged Care that you are looking to change providers. Please note – this is not your official notice to your current provider.

Tell your current Home Care Provider that you are stopping using their services. You will need to give at least 14 days’ notice and may need to give up to 30 days’ notice. Include in your correspondence your preferred end date, bearing in mind the start date for your new Home Care Provider.

4. Start receiving Home Care Services

You will need to sign a new Home Care Agreement, a new care plan and budget with your newly appointed Home Care Provider.

Don’t forget to consider the exit fees that your current provider may charge you. They can only charge you their published exit fees if you have unspent funds available.

Oxley Home Care doesn’t charge establishment fees, re-assessment fees or exit fees.

5. Don’t worry – Oxley Home Care can help you.

If you’ve decided that you would like Oxley Home Care to be your new Home Care Package Approved Provider, we can take care of all of the paperwork. We will move you from your current provider seamlessly.

Simply call us on 1300 993 591 and we will move you across to Oxley Home Care.

Have you received your Home Care Package?

To be eligible for a subsidised Home Care Package, you must register with My Aged Care and be assessed by an Aged Care Assessment Team (ACAT). We can help you do this by phone or online. Please feel free to call us on 1800 221 039.

We also offer a range of Private Care options that can be tailored to suit your needs.
