Oxley Home Care becomes a DVA Community Nursing Provider
Oxley Home Care have been contracted by the Department of Veteran Affairs (DVA) to provide DVA Community Nursing Services to clients living in Sydney Metropolitan area.
DVA provides veterans and war widows/widowers (entitled persons) with access to a range of quality health care and related services, including community nursing services, at DVA’s expense.
The DVA Community Nursing Program provides home community nursing services for entitled persons to meet their assessed clinical and personal care needs.
The aim of DVA’s Community Nursing Program is to enhance the independence and health outcomes of the entitled person by avoiding early admission to hospital and/or residential care through access to community nursing services. These community nursing services are delivered by registered nurses, enrolled nurses and nursing support staff.
If you are a Gold Card Holder or Veteran White Card holder living at home and have been assessed as needing DVA Community Nursing home care assistance, you may be eligible for support through DVA Community Nursing.
To receive DVA Community Nursing, the veteran, war widow/widower must receive a referral from their Doctor, the Hospital Social Worker or Hospital Discharge Planner.
Following a referral from a Doctor, Hospital Social Worker or Hospital Discharge Planner, our Registered Nurse will arrange a time to complete a comprehensive assessment.
What level of assistance is available?
The duration and frequency of support will be dependent on the outcome of the comprehensive assessment.
What type of support will I get?
The types of DVA Community Nursing services that may be provided include:
- Case Management
- Personal Care
- Wound Care
- Incontinence management
- Chronic disease management
- Palliative care
- Pain management
- Making referrals to other Allied Health, and other community care services as needed
DVA Community Nursing is funded by the Australian Government through the Department of Veterans Affairs. There is no cost to program recipients.
