Collection of Personal and Sensitive Health Information
- The personal and sensitive health information you provide here will be used by Oxley Home Care to perform and record the following activities listed;
Holistic Client Assessment, Cognitive Assessment, Falls Assessment, and other assessments to assist in the development of care plans and implementation of support services.
Care Plan Assessment and Care Plan Reviews to assist with your care.
Conduct Environmental Risk Assessments of your residential home to ensure the workplace health and safety of the staff and personnel who visit your home when providing your care.
Wound Care Management including the taking of photographs and measurements to monitor the progress of any wound healing process.
Oxley Home Care may also collect personal information and sensitive health information from other allied health providers, health specialists, general practitioners, government bodies including Medicare and My Aged Care, and client advocates and representatives, including next of kin to assist with providing your care.
If you decide not to disclose your personal and sensitive health information to us, we will be unable to provide you with care and services listed in this Collection Notice.
All personal and sensitive health information that you provide maybe be disclosed to others including;
Next of kin and or Enduring Guardians.
Service providers and contractors that Oxley Home Care engages to assist in your care including but not limited to allied health providers, nursing agencies, health specialists and general practitioners.
Ambulance and emergency services, hospitals and other health care facilities, aged care assessment teams and other relevant government agencies.
Product suppliers including but not limited to pharmacists and consumables suppliers for items to assist and support your care needs including hire services for equipment and aides.
